NDANDO HOUSE

is a structured and supportive living environment which operates

24 Hours A Day - 7 Days A Week

Target Population:

Individuals enrolled in the HCBS-AMH program must by 18 years of age or older and require the intensity of services provided by HCBS-AMH in order to establish and/or maintain stability in their preferred community, improve functioning, prevent relapse to an acute inpatient level of care and maintain residence in the community.

This is evidenced by the following:

Diagnosis of serious mental illness (SMI); Extended tenure (three or more cumulative or consecutive years) in an inpatient psychiatric hospital during the five years prior to enrolling; Clinical eligibility based on the HCBS-AMH Uniform Assessment (UA); Financial eligibility as indicated by the UA; Services provided in the program can reasonably be expected to improve the condition of the individual or prevent further regression; and Individual is not accessing these services by any other means, including enrollment in Long-term Services and Supports (LTSS), Community Living Assistance and Support Services (CLASS), Deaf Blind with Multiple Disabilities (DBMD), Home and Community-based Services Waiver (HCS), Texas Home Living Waiver (TxHml), or STAR+PLUS HCBS Waiver.

Criteria

Adult (18 years of age and older) with a diagnosis of serious mental illness (SMI); and have a history of

1. Long-term psychiatric hospitalization(s);

2. Frequent misdemeanor arrest and psychiatric crisis; and

3. Frequent emergency department (ED) visits and psychiatric crisis. 

If you experience any of these things below:

  • Can’t eat or sleep
  • Can’t perform daily tasks like going to school
  • Don't want to hang out with your friends or family
  • Don’t want to do things you usually enjoy
  • Fight a lot with family and friends
  • Feel like you can't control your emotions and it’s affecting your relationships with your family and friends
  • Have low or no energy
  • Feel hopeless
  • Hear voices
  • Smoke, drink, or use drugs
  • Have random aches and pains
  • Wants to harm yourself or others
  • Feel confused, forgetful, edgy, angry, upset, worried, or scared
  • Can't stop thinking about certain things or memories
  • Feel numb or like nothing matters

Clinical Approach

The NDANDO HOUSE will implement the Adult Needs and Strengths Assessment (ANSA). The ANSA as the standardized assessment tool is an open domain tool for use in service delivery systems that address the mental health of Adults and their families.
